Job Requirements

Must be at least 18 years old
high school diploma or equivalent preferred

Job Duties

Maintains and updates facility orientation material for all new admissions
facilitates the completion of the patient/resident expectation questionnaire for all new admissions
inspects and ensures all subacute rehabilitation rooms are properly cleaned, maintained, and beds are made ready for new admission
welcomes new residents and their families into the facility, introduces residents to the staff and provides a tour of the facility ensuring all questions are answered
communicates daily with residents to ensure their needs are met
identifies any resident concerns and ensures prompt resolution
maintains a daily log with questions, concerns raised and follow up taken on steps to rectify any identified concerns
