Job Description
Tallahassee Memorial HealthCare (TMH) is a private, nonprofit community-based healthcare system serving a 22-county region in North Florida and South Georgia. TMH stands as the region's foremost healthcare leader and an employer of choice with over 6,000 dedicated colleagues representing the diverse communities it serves. With a strong commitment to delivering advanced and compassionate care, TMH operates a 772-bed acute care hospital equipped to handle a broad range of medical needs. This includes being the region's only Level II Trauma Center, Primary Stroke Center, Level III Neonatal Intensive Care Unit, and Pediatric Intensive Care Unit. The hospital also boasts the most advanced cancer, heart and vascular, orthopedic, and surgery programs in the Panhandle region.
Beyond its flagship hospital, TMH encompasses a psychiatric hospital, a network of specialty care centers, six residency programs, and over 50 affiliated physician practices, all collectively working to provide comprehensive healthcare services to patients across the region. The open position is a vital role within TMH’s Case Management team, reporting directly to the Manager of Case Management. This role primarily focuses on coordinating and delivering governmental forms to patients, ensuring they receive necessary documentation accurately and timely. In addition, the role supports patients by arranging transportation and aiding in other duties as needed, making it a crucial part of the patient support infrastructure. The position requires a minimum of two years of experience, proficiency with computers and Microsoft Office, and a high school diploma or equivalent. No specific certifications or licenses are required for this role, allowing professionals with a background in healthcare support or related fields to contribute effectively.
