About Sacramento Covered

Sacramento Covered, now known as Community HealthWorks, is a nonprofit organization dedicated to enhancing the health and well-being of individuals and families in the Sacramento region. Established in 1998, the organization has evolved to offer a comprehensive suite of services aimed at improving access to healthcare and related resources.

Health Coverage Assistance

Community HealthWorks assists individuals in obtaining health, dental, and nutrition coverage through various programs, including Medi-Cal, Covered California, Access for Infants & Mothers (AIM), Kaiser Permanente Child Health Plan, and CalFresh.

Their Health Navigators work closely with families and individuals to explore healthcare options, enroll in affordable coverage, find suitable healthcare providers, schedule medical appointments, and address specific health barriers such as transportation and language interpretation.

Patient Navigation

Operating within hospital emergency departments, Patient Navigators help patients establish a primary care home for all their health needs, aiming to keep them healthy and reduce the chances of readmission. They provide thorough follow-up with each patient to ensure recovery.

They work closely with health plans, primary care physicians, specialists, nurses, and clinics to address social and medical needs for improved care coordination.

Community Health Worker Program

As a leading partner of the City of Sacramento's Pathways to Health + Home program, Community HealthWorks' team of Community Health Workers provides individuals experiencing homelessness with intensive outreach engagement and care coordination services to ensure they receive the wraparound services they need for ongoing care and stability.

They connect individuals in the program to primary health and behavioral health services, alleviate barriers like transportation, provide advocacy and comprehensive follow-up, and assist with connecting to needed services such as housing, DMV assistance, and social security.

Additional Services

Beyond healthcare coverage, Community HealthWorks offers support in finding doctors and dentists, scheduling health appointments, arranging transportation to health appointments, providing language interpretation services, and connecting individuals to other health-related resources.

All services are free and provided in multiple languages to ensure accessibility for the diverse Sacramento community.
