Serving over 40,000 individuals and families across 10 counties, the foundation offers a comprehensive range of programs and services tailored to meet diverse needs.

Behavioral Health Services

The foundation provides evidence-based counseling services to children, families, and adults, including individual, family, and group counseling sessions.

These services address various mental health challenges and are designed to meet the unique needs of each individual and family.

Early Childhood Education & Development

Easterseals Blake Foundation offers programs aimed at helping children and adults learn and re-learn basic functions, master skills needed to develop and thrive, and remain active as they age.

Support for Adults with Intellectual & Developmental Disabilities

The foundation offers a range of training, placement, and related services to help adults with intellectual and developmental disabilities prepare for the workforce.

Psychiatric Care & Medication Management

Easterseals Blake Foundation provides psychiatric care and medication management services to support individuals in managing their mental health effectively.

Adult Supported Employment Programs

The foundation offers supported employment programs for adults with intellectual and physical disabilities.

Supported employees work closely with job coaches to gain competitive work skills in their field of choice and receive expertise in adaptive equipment and workplace modifications.

Residential Services & Community-Based Residential Homes

Easterseals Blake Foundation operates twenty community-based small group residential homes for adults and children with moderate to severe intellectual and/or physical disabilities.

These homes provide individualized support and treatment strategies with person-centered service plans.

Attendant Care & Respite Services

In-home assistance is available for individuals with intellectual or physical disabilities who live independently or with their family or caregiver.

The services focus on personal and physical care needs, basic home management, supervision, community integration, and relief for primary caregivers.

Individually Designed Living Arrangements

The foundation provides person-centered residential support for adults with intellectual and physical disabilities living in their own homes.

Coaching and assistance include personal care, budgeting, bill paying, menu planning, grocery shopping, home upkeep, transportation, and community involvement with support from a few hours weekly up to 24-hour care.

Aviva Resource Closet

Aviva Children's Services helps children in foster care by providing essential items like socks, underwear, diapers, and wipes.

This program has expanded to Yuma and Cochise counties, serving children through collaboration with Department of Child Safety and licensed workers.

SAGE Ceramics

SAGE Ceramics is a unique supported employment program for adults with intellectual and developmental disabilities.

Employees design and manufacture high-quality, handcrafted ceramic gifts while gaining occupational skills related to the retail and wholesale gift industry.

Blake Behavioral Health

Easterseals Blake Behavioral Health serves children, adults, and families across nine counties of Southern Arizona with trauma-informed counseling, parenting education, Applied Behavioral Analysis treatment, substance-use counseling, employment services, and medication management.

These services aim to support individuals and families in achieving their goals and improving quality of life.
