Job Requirements

  • Understanding regulations and standards applicable to hospice
  • Knowledge and understanding of hospice agency functions
  • Demonstrated knowledge of dying patient and pain control measures
  • Genuine interest in and compassion for patients and families dealing with end-of-life issues
  • Understanding hospice philosophy and issues of death and dying
  • Ability to be flexible, organized, and function under stressful situations
  • Ability to communicate effectively in English, both verbally and in writing
  • Excellent interpersonal skills
  • Excellent writing and presentation skills
  • Knowledge of general modalities and scope of practice within state of agency operation
  • Self-directed and able to work in the field with minimum supervision
  • Valid driver’s license and reliable transportation
  • Proof of automobile insurance (MVR checked)
  • Proof of current CPR certification
  • Able to pass criminal background check and national sex offender clearance

Job Duties

  • Provides care ordered by the physician as indicated by the plan of care including baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing patient, feeding, and transferring patient
  • Takes accurate vital signs
  • Describes the activities of daily living and demonstrates techniques to assist the patient to meet these activities
  • Participates in case conferences to discuss interdisciplinary team responsibilities and patient progress
  • Notifies appropriate licensed staff when patient complains of pain
  • Positions patient in bed with proper body alignment
  • Assists patient with ambulation as necessary
  • Performs patient care in a safe environment to reduce medical errors
  • Supports cultural practices that do not harm the patient or interfere with treatment
  • Identifies and communicates findings to Case Manager and Clinical Manager
  • Provides clear documentation in medical records related to reimbursement and agency policies
  • Works as part of the interdisciplinary group
  • Participates in agency mandated activities as requested
